چکیده

PURPOSE We report a patient with abnormal head posture following ocular blunt trauma. METHODS This is report of a case that despite findings compatible with diagnosis of left superior oblique (SO) palsy, the patient acquired an ipsilateral (left) head tilt. The interesting observation in our patient was reduction of left hypertropia and consequent less diplopia with ipsilateral head tilt. RESULTS After blunt trauma, our patient adopted paradoxical left head tilt and consequently less diplopia despite acquired left SO palsy. Left inferior oblique myectomy resulted in significant improvement of patient's strabismus and abnormal head position. CONCLUSION Traumatic SO palsy may present with paradoxical head tilt.

The influence of head tilt on ocular torsion in patients with superior oblique muscle palsy.

INTRODUCTION Patients with superior oblique muscle palsy often assume a compensatory head tilt to the contralateral side of the affected eye. This tilt serves to decrease the hypertropia and, in theory, might decrease the excyclotropia. METHODS A prospective investigation was undertaken of the effect of forced head tilt to the right and left on torsion and the hypertropia in patients with uni...

Superior Rectus Muscle Recession for Residual Head Tilt after Inferior Oblique Muscle Weakening in Superior Oblique Palsy

PURPOSE Residual head tilt has been reported in patients with superior oblique muscle palsy (SOP) after surgery to weaken the inferior oblique (IO) muscle. The treatments for these patients have not received appropriate attention. In this study, we evaluated the superior rectus (SR) muscle recession as a surgical treatment. METHODS The medical records of 12 patients with SOP were retrospectiv...

Does Decreased Static Ocular Counter Rolling Account for Bielschowsky Head Tilt Test in Unilateral Superior Oblique Palsy?

Purpose To understand the relationship of static ocular counter rolling (s-OCR) and clinical manifestations in acquired unilateral superior oblique palsy subjects during the Bielschowsky head tilt test. Methods Nineteen subjects that were diagnosed with acquired unilateral superior oblique palsy were included. Fundus photographs were obtained at different head tilt angles to evaluate static o...

Effect of strabismus surgery on torticollis caused by congenital superior oblique palsy in young children

PURPOSE To evaluate the outcome of strabismus surgery for congenital superior oblique palsy (SOP) in relation to correction of head tilt and hypertropia. The cohort of patients mainly involved very young children. This is the first study to use a standardized instrument to objectively measure torticollis before and after surgery. MATERIALS AND METHODS A non-comparative interventional case ser...

Bielschowsky head-tilt test--I. Ocular counterrolling and Bielschowsky head-tilt test in 23 cases of superior oblique palsy.

We have measured the amplitude of ocular counterrolling (OCR) and the change in vertical deviation in the Bielschowsky head-tilt test (BHT) in 23 cases of unilateral superior oblique palsy. OCR was measured with a photographical method, using limbal, conjunctival vessels as landmarks. Average OCR of the healthy eye was 5.4 +/- 2.4 (SD) deg either side, at 45 deg of body-tilt. BHT and OCR (of th...
